If you are having problems with the online spot request program, you can use this form to fill in the necessary information. Simply print out this form, fill out the appropriate information, and then call or fax us the information. The form is set up similar to the form that you would fill out when making a request online. Please do not forget to call us if you fax this form to the National Weather Service in the Twin Cities. The telephone and fax numbers of our office are available through the Minnesota Interagency Fire Center (MIFC) or your agency.
